HSS
HSS is a Medical Assistance (MA) benefit program overseen by the Minnesota Department of Human Services (DHS). We provide three types of services that may include housing transition, sustaining and/or consultation services.
Who do we support?
We support adults facing disabilities, encompassing mental illness and substance use disorder, as well as seniors, in planning for, discovering, transitioning into, and maintaining affordable housing.
Housing Transition Services may include:
* Helping the person think about preferred housing
* Helping with the housing search and application process
* Developing a budget
* Understanding a lease
* Helping to pay for moving expenses up to $3000 annually
Are you eligible?
Any adult residing in Minnesota, who has Medical Assistance coverage, is 18 years of age or older, with a documented disability or mental illness & is experiencing housing instability.
